I understand that Linden Free Public Library will seek to eliminate exposure and risk to myself/ my child to disclosed allergens, but agree that Linden Free Public Library assumes no liability for adverse reactions for food consumed or items that myself/ my child may come in contact with while participating in this program.

By registering for this event, I hereby give my consent for the Linden Library to use my photograph or my likeness to be used in its publications, including its website and social media.   I release them from any expectation of confidentiality for myself and those I have registered for this event. I attest that my parent or guardian has signed the Unattended Minors Release form and is on file with the Library.
